2. Start with a blank document or search in the search box for ‘form’
3. If you go to ‘search’ choose a template the general type of format you want. It will save time.
4. Open the developer tab
5. Click wherever you want to add your content or questions.
6. Select the desired form control (e.g. checkbox, pictures, text, etc) to add it on the survey.
7. Enter any questions or text.
8. Keep repeating the previous step till you complete your survey.
9. Select the review tab, select ‘Protect’ and then choose ‘Protect Document’.
